1- Program Registration
The first step in the process is to pay for the program registration.
This is a one-time, non-refundable fee of $125.00. This payment can
be made through PayPal or you may mail a check or money order to Choose
2 Change.
On receipt of payment confirmation from Paypal (which only takes a
few seconds), you will be redirected to the Program Registration page
where you will provide us with your personal information and a brief
summary of your situation. After you submit your information, a Registrar
will contact you within 24-hours to set up an assessment and counseling
planning session between you and your assigned counselor.
Note: An assessment and counseling plan is not required for the
Educational Programs.
2- Assessment and Counseling Plan
You and your counselor will complete a 1-hour telephone assessment.
After the assessment, you and your counselor will develop a counseling
plan . The counseling plan becomes the roadmap for your counseling process.
The counselor will forward a copy of your treatment plan to the Registrar.
The Registrar will contact you by e-mail to arrange for payment of your
program.
3- Account Setup
Upon receipt of payment, your account will then be set up for you on
the choose2change.net Online Counseling system. You will receive
an e-mail outlining the steps for you to sign onto the system and to
how to use the system.
4- Work Your Lessons
You will work on the lessons assigned to you by your counselor.
Each lesson helps you move one step closer to you goals. You are
expected to complete an assigned number of lessons each week for
the duration of your treatment program.
5- Counselor Review
When you have completed a lesson, you will submit the lesson for
review. Your counselor will review your answers and offer suggestions
or comments to clarify the lesson. You will then be given an opportunity
to respond to the counselor's feedback and submit your responses
to the counselor's questions or comments a second time.
When your counselor completes his/her second review, he/she will
write a lesson summary and lock the lesson. Once the lesson is locked,
no one can make any further changes to the lesson. The lesson is
still available to both the you and your counselor for review at
any time during the program and for up to one year after you have
completed your program.
6- One-on-one sessions
After completing your assigned lessons in a given period, you will
meet with your counselor for an online or telephone one-on-one session.
In these sessions, you and your counselor will review your progress,
update your counseling plan and schedule your next one-on-one session.
7- Certificate of Completion
A Certificate of Completion will be issued upon completion of your program.
